Official transcripts are notarized by an Oakwood official. Official transcripts are null and void if not delivered directly by Oakwood. Therefore, Oakwood does not issue official transcripts to students or families. Oakwood will send official transcripts to people or entities requiring an official transcript.
 
Unofficial transcripts will only be emailed.

I authorize Oakwood School personnel to release my transcript and GPA to colleges, universities, scholarship, athletic, internship and summer programs. Pursuant to the State of California Civil Code, Sections 1633.1-1633.17, by typing in my name in the name block below, I am electronically signing and consenting to the provisions of the form above. Further, in electronically signing, I certify that I am a graduate, a student or a parent/legal guardian of the student. The student or the parent/legal guardian of the student agrees that the above authorizations and consents shall continue in effect from year to year as long as the student is a student at Oakwood School, unless I subsequently notify the Director of College Counseling or the Registrar in writing that these on-going authorizations and consents have been terminated.

Under FERPA, a student 18+ must request their own transcript. A parent/guardian may not do so.
If the student is requesting their own transcript, write "Self."
