In order to volunteer indoors on the Oakwood campus, individuals must be either be vaccinated against COVID-19 or take a rapid test the day prior to their volunteer shift.

If unvaccinated, you are required to take a rapid test the day before you are scheduled to be on campus. The test must be uploaded by 5pm the day before.

If you do not have access to a test, you must come to the Front Office 30 minutes prior to your scheduled volunteer time to be tested by Oakwood Staff.

Field Trip Chaperone (including drivers)

Field trips are a rich part of the Oakwood experience. Being a chaperone on a field trip is a wonderful way to be involved, and your help is greatly appreciated. Chaperoning students is a significant responsibility and carries with it certain requirements, which are outlined below.

Please keep in mind that all decisions are made with the safety of the children in mind.

  • Chaperones will be confirmed prior to the field trip by the teacher. Only pre-confirmed chaperones are allowed on field trips.
  • No additional children or adults may accompany the chaperone. We ask chaperones to please make other arrangements for siblings so that they may be fully focused on the Oakwood students that they are assigned to chaperone.
  • Chaperones are to remain with their assigned students and group at all times. Students may not switch groups. Chaperones must accompany children when they use the restroom.
  • To ensure the safest supervision of our students, chaperones should refrain from cell phone use and texting while on field trips. If there is a serious emergency, the teacher or chaperone should call 911.
  • For non-extreme situations, the teacher or chaperone should notify the school office at (408) 782-7177.
  • Unless specifically authorized by the teacher prior to the field trip, there are to be no visits to stores or gift shops or purchases/serving of unauthorized food or treats (Jamba Juice, ice cream, sodas, etc.).
  • Chaperones must remain alcohol, drug, and tobacco-free while on field trips.

Parent/Guardian Driver

Thank you for volunteering to drive students. This is a significant responsibility and carries with it certain requirements which are outlined on this form. Please keep in mind that all decisions are made with the safety of your child in mind.

To ensure the safest supervision and transport of our students, it is requested that drivers refrain from cell phone use while driving. If there is a serious emergency call 911. For non-extreme situations notify the school office at (408) 782-7177.

Unless specifically authorized by the school, drivers should not make any stops. This includes stopping for unauthorized food or treats (Jamba Juice, ice cream, sodas, etc.) on the way to or from the location.

If transporting students, the school requires that a parent driver not use a cell phone, even with a hands-free device, and only use a cell phone in the case of an emergency.

In addition, Oakwood has a strict policy regarding the showing of videos and video games while transporting students:

  • ECE & Elementary – Only G-rated videos may be shown in vehicles transporting students. Only E-rated video games may be used in vehicles transporting students.
  • Middle & High School – Only G & PG (note – NOT PG-13) rated videos may be shown in vehicles transporting students. Only E-rated video games may be used in vehicles transporting students.

Oakwood’s insurance carrier may require that a Department of Motor Vehicle Authorization check be conducted on each person who wishes to transport students. Anyone wishing to drive students is required to maintain the following: a current Drivers License (mandatory), automobile insurance with minimum liability limits of no less than $100,000 each person/$300,000 each accident (mandatory), and your vehicle must be equipped with a seat belt for each passenger.

If your driving record indicates more than two moving violations in the past three years or reflects a violation for reckless driving or driving under the influence of drugs or alcohol, we ask that you not sign up to participate.

If you want to be eligible to transport students, please complete and submit the information below. A new form must be submitted each school year.
